We are the
Southeast Partnership for Health, and we have more
than 100 years experience in healthcare and health
support.
Mission
The overarching mission and goal of the
Southeast Partnership for Health Center
of Excellence is
to establish a high-quality, accessible, comprehensive, culturally
competent, and fully integrated HIV service program that
extends the quality and length of life for low-income HIV
infected and affected African American San Francisco residents
with a focus in the southeast corridor.
Philosophy
The shared philosophy of
our partnership includes:
- A strong belief that every
individual has the right to high-quality, comprehensive, and culturally responsive
health and social services regardless of economic or life circumstances.
- A respect for the value,
worth, and dignity of every individual served through the
program, reflected in a non-judgmental, harm reduction-based
approach to services which meets each individual at the
present point in their lives, and provides care and services based on individual
needs and preferences.
- A commitment to providing
a comprehensive range of tailored services for each client
that continually facilitates accessibility of services
regardless of client condition or need.
- A belief in the importance
of strong trusting relationships developed over time between clients, their
caregivers, and their care coordinators, particularly for
individuals who are disenfranchised, alienated from, or
mistrustful of medical and social service systems.
- A belief in the centrality
of social and family relationships in the lives of persons with HIV, and of
the incorporation of family members, friends, loved ones,
and caregivers in the spectrum of comprehensive HIV care.
- A commitment to the idea
that in order to improve community health, care systems
must involve entire communities in efforts to achieve,
comprehensive, holistic solutions to social, economic,
and behavioral problems .
